- Red flags bypass step therapy: progressive deficit, bowel/bladder changes, cancer history, fever with back pain, trauma
- Duration of symptoms: most payers require 4–6 weeks for non-emergent radiculopathy
- Specific ICD-10 pairing: lumbar radiculopathy (M54.4x) better supported than non-specific low back pain (M54.5)
